Eligibility

Individuals must: A) Be 18 years of age or older B) Meet Serious Mental Illness (SMI) criteria as defined in Mental Health Bulletin OMH-94-04 Serious Mental Illness: Adult Priority Group C) Demonstrate moderate to severe functional impairment that interferes with or limits performance in one of the following life domain areas: Educational; Social; Vocational; Self Maintenance D) Be recommended by a licensed practitioner of the healing arts

Administration and Intake: Monday through Friday, 8:30am to 4:30pm Call for later appointments.

Mental Illness

Provides Peer Support Specialists (individuals who are self-identified current/former consumers of mental health services) who review participants’ progress in identifying and developing natural supports and social networks, identify and develop strategies to increase self-help and self improvement, and identify any additional areas that the participant feels they need additional support with in order to aide in their recovery.
