Eligibility
Serves youth and young adults from birth to 21 years old.
Youth must have a mental health diagnosis and be considered at imminent risk of out of home placement.

Office Hours: Weekdays, 8:30am to 4:30pm
Application process
Accepts agency referrals from school districts, Juvenile Probation, Children and Youth services, MH/MR and managed care. Call for additional details.
