Monday through Thursday, 8:00am to 8:00pm Friday, 8:00am to 5:00pm

Call for an appointment. Must have funding/insurance information ready. Evaluation must be completed prior to the start of treatment. Evaluation includes completion of required forms. Be prepared to complete paperwork at evaluation, or have something with you who can assist with completion of forms.
