Application process

Visit a Senior Center in person on or after the announced Distribution Day. Call for additional information. Visit the intake website to find a list of Senior Center locations. Individuals who cannot pick up their own vouchers may designate a proxy to accept vouchers on their behalf. Proxy forms are available online and at Senior Centers.
