Home-based services available to assist consumers who are able to live independently but having difficulty meeting their activities of daily living.

  • DAILY LIVING ASSISTANCE TASKS: bathing, dressing and grooming, doing laundry and getting groceries.
  • RESPITE CARE: sole caregiver of a consumer is able to leave the home for short periods weekly for business or recreational purposes.
  • HOME SUPPORT SERVICES: assistance with laundry and grocery shopping for up to 2 hours every other week. Note: laundry and grocery shopping are private pay.
  • Trained care assistants from contracted Home Health Agencies provide the services authorized by the care manager.

    Application process

  • Care managers complete an in-home assessment to determine the areas of need with their activities of daily living, to develop a care plan, and to authorize the services needed to help the consumer remain living independently.

  • Fee

    Varies. Laundry and grocery shopping are private pay tasks.

    Eligibility

    * York County residents in a non-institutional setting. * 60 years of age or older. * Have a physical, mental, or emotional disability that requires assistance in order to remain living independently in their home. * The consumers appropriate for this service generally require less than 5 hours per week assistance. * Home bound to the extent that they are unable to leave their residences without assistance and/or supervision. * Lack adequate informal support from family or others.

    Maintains the mission of York County Human Services Division to provide equal access to services that support the safety and well-being of all eligible residents in a timely and cost-effective manner.
