A) 55 years old or older
B) Meet the level of care as determined by the State
C) Live in an area served by Senior LIFE
D) Able to live safely in the community
Office Hours and Day Program: Monday through Friday, 8:00am to 4:30pm
LIFE Services are available to members 24/7

No costs for individuals who are Medicaid eligible. Offers private-pay option for individuals who are not eligible for Medicaid.
