Serves as the central hub for intake and registration for supports coordination for eligible individuals. Provides information and answers questions about available services and supports.

Application process
Apply online through the Intake Website (linked below) or call for assistance. If initial screening deems individual likely eligible, staff will guide the individual (or parent/guardian) through the process of registration and selection of a case management agency that will provide service (Supports Coordination Organization).
Fee
Referral services provided free of charge. Support Services are waiver-funded.
Eligibility
Allegheny County residents with Intellectual Disability (ID), Autism, Developmental Disability (DD) prior to age 9, and/or Medically Complex Condition (MCC) diagnosed prior to age 21. Must also meet additional Pennsylvania Office of Developmental Programs (ODP) criteria.
