Individuals age 18 to 21 under the care and custody of a child welfare agency; Not actively suicidal, homicidal, active substance abusing or in need of in-patient services

Provides supervised housing/residential transitional living services to older adolescents in care of the child welfare or juvenile justice system. Services include:
  • Community group living
  • Life skills (individual/group training)
  • Assistance in developing and completing and individual plan for independence and permanency
  • Individual apartments: Case Management Services provided
