A) Adults age 60 and older B) Individuals 18 to 59 with a physical disability that will last 12 or more months C) Resides in Cumberland County (Services can transfer to other PA counties) D) Meet financial requirements

Monday through Friday, 8:00am to 4:30pm
Application process
All Waiver Enrollments are managed by the "PA Independent Enrollment Broker" (IEB) - consumers must call the IEB to enroll - 1-877-550-4227.
