Individual/Head of Household must be literally homeless and have a mental
health disability, able to live independently. Cannot accept direct
referrals.

Cannot accept direct referrals. All households experiencing homelessness must call 211 and be assessed, placed on the By Name List in the Coordinated Entry System. We are required to pull from the Coordinated Entry System only for this program.
