Eligibility
Individuals must meet all of the following criteria: A) Diagnosed with end-stage renal disease B) Residents of Pennsylvania for at least 90 days prior to the application date, or individuals who demonstrate intent to live permanently in Pennsylvania C) United States citizens or legally authorized immigrants D) Individuals with income within the guidelines of the Pennsylvania Department of Health

For information concerning CRDP enrollment, contact the customer service hotline at 1-800-225-7223 and ask for an eligibility specialist. Eligibility specialists are available from 8:30am to 5:00pm, Monday through Friday.
Fees
Prescription co-pay. Patient contributions based on sliding scale.

Provides life-saving care and treatment for adults with end-stage renal disease (ESRD). Assists with costs related to renal dialysis in all modes, kidney transplantation, medical management, inpatient and outpatient services, certain prescription medications, home dialysis supplies and equipment, certain physician fees, and limited patient transportation.
