Eligibility
A) Individuals aged 18 to 59
B) Physically and/or mentally disabled (temporary medical conditions excluded)
C) Income below $1,428 per month and/or has Access Card
D) Unable to prepare adequate meals for self
E) Lives alone or with someone unable to prepare meals or grocery shop,
F) Homebound (to be assessed and at the discretion of the Executive Director)
