Caregiver Support Program
Provides reimbursement for care giving expenses for people caring for parents, spouses, children or relatives that live at home. Benefits provided include: financial reimbursements, adult day care, skilled nursing and household expenses, home modification or assistive devices such as ramps and wheelchairs. Information and referral on resources, respite care, benefits counseling, caregiver training, and care coordination may also be available.

Eligibility
A) Caregiver of someone age 60 or older. B) Financial eligibility determined by proof of annual household income. The services being provided can not be completed by a family member. Also can assist grandparents 55 years or older who have legal, custodial or guardianship of their biological grandchild.
