Application process

To begin the intake process, please call our office. Walk-in intakes are also welcome at our Wyomissing location. Please allow approximately 20–30 minutes to complete the intake. Individuals applying for the sliding fee scale will be scheduled for an appointment with a staff member. Household income verification is required and may include the most recent two months of pay stubs or the previous year’s tax return.

Fees

Accepts most major health insurance plans, including Medicare and Medical Assistance, and participates with many employee assistance programs (EAPs). A sliding fee scale is available for eligible individuals without insurance.
