Application process
Once a completed referral is received, we contact the referred participant to schedule and intake in his/her home or somewhere in the community, wherever he/she is most comfortable. The case manager will complete an assessment to determine if he/she meets the criteria for case management and what level of care is needed. Once a supervisor approves the case manager's assessment, registration paperwork is completed.
