Eligibility

A person is eligible to transition from a nursing facility if they are 21 years of age or older, express to the social services department of the nursing facility that they desire to transition, and have an identified barrier to transitioning.

Nursing Home Transition is a comprehensive coordination of services and plans development to provide alternatives to institutional living to individuals aged 18 and older. Through outreach and service coordination with nursing facilities and residents, the purpose is to relocate from institutions to community living with any needed services in place.
