Administers coordinated care at home by the Home Health team when treatment from multiple healthcare disciplines is required. Offers specialties in wound care/wound VAC, IV infusion, cardiac management/post CABG/CHF/ post CVA, post surgery care/joint replacement/amputation, anticoagulation management, ostomy care, foley care, diabetic management, and chemotherapy administration/disconnect.
