Individuals may self-refer, or a medical, behavioral health, or social services provider may complete the initial screening/referral form.
Participants who are pregnant and have or are eligible for medical assistance/Participants who have gestational diabetes and have any insurance/Participants who are parenting 0-18 months and have or are eligible for medical assistance are all eligible. Please see website for any other eligibility provided through a funder.
Office Hours: Monday through Friday, 8:00am to 4:30pm. Service hours may
vary.
