Eligibility

Visual Requirements (must have at least one): A) Visual acuity of 20/70 or poorer in better-seeing eye that cannot be corrected or improved with regular eyeglasses B) Corresponding loss of visual fields C) Progressive sight threatening eye diseases, such as diabetes D) Diagnosis of a degenerative eye or sight disorder E) Age18 or older

Monday through Friday, 8:00am to 4:00pm
