Individuals must have a Mental health diagnosis, be homeless and willing to live with others. Documentation is a must. For Families, one member of the household must be disabled and be able to provide documentation, as well as be homeless.
Monday through Friday, 9:00am to 4:00pm
Referrals for services are made through Coordinated Entry System.
