Application process
A referral is completed by the referral source and participant (the participant may self-refer). The referral needs to be signed by a Licensed Professional of the Healing Arts - (MD/DO, Licensed Psychologist, Nurse Practitioner). There also needs to be a psychiatric evaluation from within the past year attached to the referral.
