Eligibility
A) U.S. citizen or a qualified Non-citizen B) Have a Social Security Number C) Be 21 years of age or older with a physical disability D) Income and resource eligible E) Meet the required level of care for a nursing facility
Hours
Monday through Friday, 8:30am to 4:30pm
