You will meet with a Blended Case Manager who will partner with you and your family to achieve your goals. You will meet with your Blended Case Manager at your home, at PATH or in the community. You will receive help from your Blended Case Manager to obtain the services you and your family need such as housing, health insurance, and appropriate school placement. You will receive help from your Blended Case Manager to obtain behavioral health, substance abuse, or other medical treatment. You and your family will receive assistance from your Blended Case Manager to effectively collaborate with your school and assure that your academic needs are met. You will be supported by your Blended Case Manager as you resolve problems and begin to develop confidence in your own abilities to meet lifeâs challenges.
