Description
Provides information about services available to seniors. Conducts a brief interview to determent the inquirer's needs in order to make an appropriate referral. Provides relevant information about community resources and referral to the needed services. Helps to match older persons with nursing homes, personal care boarding homes (assisted living), and independent housing. Provides assistance in completing applications for public benefit programs, such as PACE, and Property Tax/Rent Rebates.
