Rehabilitation Services
Provides funds for services through a contracted provider to help individuals with a traumatic brain injury (TBI) live independently in their homes and communities. Services includes pre-enrollment assistance, pre-admission assessment, service plan development, rehabilitation and case management.

Application process
To enroll call 717-772-2762 or 1-866-412-4755. After answering a few simple questions, the Enrollment Specialist will mail a Head Injury Program application to you. You can also expect to be contacted by a Pre-enrollment Assistance Coordinator who will offer you individualized support in completing the application as well as assist in identifying any other statewide resources.

Eligibility
A) U.S. citizen B) PA resident at time of injury and application C) Traumatic brain injury after July 2, 1985 D) Have exhausted all resources for head injury care E) 21 years of age or older F) Meet income guidelines G) Complete an application H) Be assessed to determine that needs can be addressed by the Head Injury Program through rehabilitation.
