Eligibility

A) Women who are pregnant or had a baby in the past 6 months or 12 months if breastfeeding B) Infants and children under age 5 C) Fathers, grandparents, and foster parents who are legal guardian of a child under age five D) Gross household income 185% or less of federal poverty income guidelines
