Monday through Thursday, 8:00am to 8:00pm Friday, 8:00am to 5:00pm

Application process

Interested individuals should complete a HealthConnections referral form and call. Referrals can be completed by providers, community, supports, or self-referred. Upon receipt of referral, a team member will reach out to discuss the program and complete enrollment as appropriate.
