Permanent Supportive Housing
Provides supportive permanent scattered site housing to chronically homeless and disabled individuals and families. Participants are encouraged to complete a 12 month service plan, participate in weekly life skills classes, maintain savings, obtain employment or participate in educational programs as applicable, complete volunteer service, maintain health counseling where applicable and participate in regular home visitation by casework staff.
