Eligibility

• Adults from age18 to age 65 who are engaged in opiate addiction or those already on Buprenorphine and Vivitrol • Works with individuals on medical assistance, self-pay, or those eligible for county funding.

Call first for a phone intake, then scheduled for a face-to-face appointment.

Fees

No fee for individuals on medical assistance or is county funded. Self-pay clients will get fee schedule during phone intake.

Provides care coordination. Offers assessment, referral, recovery planning, care monitoring, medication monitoring, and interface with prescribing physicians and outpatient treatment programs to provide a team approach.
