Service Coordination Unlimited
SCU offers service coordination and supports coordination to individuals with various physical, intellectual and developmental disabilities, and autism. SCU facilitates the coordination of services that help people to live independently in their homes and communities. We use a person-centered approach to empower participants to make informed decisions about their care and support, helping them connect to community services that strengthen their independence. These services include home care, nursing, meals, transportation, community participation, home modifications, and more. The overall goal is for each individual to live a safe and meaningful life within the community.
