Application process

Call the Benefits Hotline or your County Assistance Office to report the loss, then complete the affidavit (available in the Intake Website linked below) and submit it to County Assistance Office. The COMPASS app is equipped for submission of the form as well. In person reporting is also available during normal business hours at each County Assistance Office. Hard copies of the affidavit are on-site.
