Stairways Behavioral Health - Blended Case Management Adult
Provides case managers who help individuals diagnosed with serious mental illness access community services needed in order to live independently and maintain mental and physical health. Case managers assist in areas such as, but not limited to: mental and physical health care, housing, education/work, daily living, income/benefits, and drug/alcohol services.

18 years old and older; resident of Erie County; diagnosis of a serious mental illness such as schizophrenia or bipolar disorder; significant difficulty independently managing responsibilities at home, at work, or in the community; recent psychiatric hospitalization or frequent contacts with Crisis Services.
