CareLink currently provides services to residents of Delaware, Chester, and Montgomery Counties in Pennsylvania, Available services vary by county and individual CareLink site.
Monday through Friday, 8:30am to 4:30pm
Call for information. Program participants are typically referred to CareLink by county government agencies, social service providers, and health care providers.
CareLink's services are primarily funded through the offices of behavior health in the counties they serve, but the Mobile Psychiatric services, Supported Living services, and Education and Employment support services are available with a private pay option to those living outside those counties.

Serves as a point of entry for people with serious mental illness to be admitted into any of the psychiatric rehabilitation services in Delaware County. Makes referrals to the appropriate services and agencies.
CareLink Community Support Services
Provides people with mental illness a progressive alternative to the revolving door of hospital care. Caring support, housing, assistance with jobs, and practical help in the community resulted in more dignity, stability, and hope.
