Individuals who are 60 years of age and older, that will benefit from an
adult day program

Application process
Individuals are advised to call and set up an appoint for a tour of the facility. Individuals will then need to complete Assessment, medical form and TB test as they are required.
Fees
Possible funding through Area Agency on Aging, Veterans Affairs, Private Pay, or Long Term Care Insurances.; Individuals should contact the agency to learn of what fees or insurance are accepted.
