Coordinates all chronic disease programs sponsored and facilitated in partnership with other agencies; acts as a team leader for Injury Prevention, Tobacco, Oral Health, Asthma, Arthritis, Osteoporosis, as well as Cardiovascular programs. Chronic disease team nurses provide information to the public by presenting educational programs and are available for individual consultation.
