substandard housing conditions. Must have a diagnosed mental health
illness and be enrolled by Service Access and Management

Assists individuals in securing financial resources, applying for pertinent documents, searching for housing and applying for housing.
In some cases, financial assistance such as first month's rent, a security deposit and/or basic household needs is also available through PATH.
