Assessments completed for individuals applying for Agency services. - Home Delivered Meals, Personal Care, Personal Emergency Response Systems, Adult Daycare and the Caregiver Support program. - Care Management is provided by trained Aging Care Managers. - Level of Care Assessments provided to individuals applying for waiver programs or facility placement.

Fee
Care Management and Level of Care Assessments are provided at no cost. Program fees may vary. Consumers may be eligible for Cost Share or an income-based fee scale. Other payment options may include services provided on donation basis (or no fee), payment through an outside agency (VA or MH/ID/EI), or private pay.
