Magellan Insurance clients who are:

age 18 and older AND
Cambria County residents AND
have mental health challenges and/or substance use disorder AND
are at risk of or currently experiencing homelessness.

Assists participants in maintaining their current housing, avoiding homelessness, or transitioning from being unhoused to appropriate stable housing. Social determinants of health are addressed weekly as a pathway to impact physical and behavioral health outcomes.
