Assesses client's needs and helps client to access community-based services and resources. Arranges for in-home support, such as meal preparation, assistance with personal care, skilled counseling, house and yard work, transportation to medical appointments, and the reassurance of a personal emergency response system.
Senior Community Services
Provides independent and meaningful living for older adults through direct services and programs in the home and community. The organization hosts senior centers in Yeadon, Folsom, Chester and Darby offering a variety of services such as a congregate meal, home delivered meals, adult education, referrals to support services, and more.
