Wednesday - Thursday, 8:00 AM to 4:30 PM
Application process
Individuals should call to schedule an appointment. A referral from a physician may be required by your insurance plan.
Fees
Individuals should contact the agency to learn of what fees or insurance are accepted. Fees will depends on the insurance coverage and eligibility for assistance. Medicaid, Medicare, and most insurance plans are accepted. There is a Payment Plan available if needed as well as an Assistance Plan for those that meet requirements for enrollment.
