Description
Provides patient care in collaboration with the patient's physician. Makes every effort to reduce re-hospitalizations in patients with CHF or COPD while helping the patient and/or caregivers become more knowledgeable in ways to deal with the disease. Also provides tele-health monitoring and utilize physician signed disease specific standing orders to help manage the condition.
