If you meet one or more of the following A) Uninsured OR not-eligible for insurance B) No primary care provider, and need assistance connecting to one C) Frequent use of emergency department for non-urgent care

Monday through Friday, 8:30am to 4:30pm, by appointment only

Spanish, Translation service available

Application process

Call 717-756-5300 or email [email protected]. No drop-in appointments; a community health worker will schedule a time and place to meet based on your location.

Fees

Community health worker service is free; Medical care may be free, on a sliding scale, or at the rate of co-pay required by insurance

Offers community health workers who assist individuals with securing health insurance options, medical appointments, including a primary care provider (doctor), or referral to free/low-cost care. Can assist with dental and vision appointments. Assists with other barriers to health care.
