Once a completed referral is received, we contact the referred participant to schedule and intake in their home or somewhere in the community, wherever their is most comfortable. The case manager will complete an assessment to determine if their meets the criteria for case management and what level of care is needed. Once a supervisor approves the case manager's assessment, registration paperwork is completed.
