A Supports Coordinator (case manager) works with each individual to develop and Individualized Service Plan (ISP) focused on what the individual needs to meet their maximum potential. Supports Coordination is available at no cost to registrants.

Eligibility

To be eligible with a diagnosis of Intellectual Disability: • Full scale IQ of 70 or below assessed by a licensed psychologist or certified school psychologist using standardized testing. • Deficits in two or more areas of adaptive functioning, which includes communication, socialization and daily living. • Diagnosis of Intellectual Disability prior to age 22 To be eligible with a diagnosis of Autism with no Intellectual Disability: • An evaluation by a licensed psychologist, certified school psychologist, psychiatrist, developmental pediatrician or licensed physician who practices psychiatry, as documented in a standardized diagnostic tool. S • Deficits in two or more areas of adaptive functioning, which includes communication, socialization and daily living. T • Diagnosis of Autism prior to age 22

Chester County Department of Mental Health, Intellectual, and Developmental Disabilities

The Department serves infants/toddlers with developmental delays, residents with serious and persistent mental illness, and residents with an intellectual disability/autism. MH/IDD uses federal, state, and county funds to contract with local agencies to provide services for eligible individuals. EARLY INTERVENTION provides supports and services at no charge to help children with developmental delays grow and develop. Services may include therapies, child development information, ideas to help the child, and planning for the child's growth and learning. MENTAL HEALTH services are for county residents who live with serious and persistent mental illness, have no private insurance for mental health care, and are not eligible for Medical Assistance. INTELLECTUAL DISABILITY serves county residents with intellectual disability. The program provides Supports Coordination at no charge for registrants, and contracts with local agencies to provide other services.
