Nurse Family Partnership
NFP nurse helps mother-to-be plan for a healthy pregnancy, prepare for delivery, and get ready to welcome new baby in a safe, loving home environment. Nurse will connect clients with health care and social services, and provide client with caring and compassionate advise to support overall health during pregnancy. During home visits, Nurse provides care, needed support and answers questions to build confidence as a mom and guidance through all the changes experienced.

Hours
In home visits with nurse on a regular basis as scheduled, usually every week or every other week before and after birth of baby. Office Hours: Monday,7:30 am to 3:30pm Tuesday, 10:00am to 6:00pm Wednesday, 9:00am to 5:00pm Thursday, 9:00am to 5:00pm Friday 7:30am to 3:30pm

Low income mothers-to-be who meet the eligibility and risk criteria. Income guidelines: 235% of the federal poverty level. Eligible if receiving WIC or Medical Assistance. Call for details.
