Offers a needs assessment evaluation to determine immediate needs and prioritize how assistance may be provided. The needs assessment is a mutually agreed upon action plan is developed to establish goals and guide the client and the case manager. Case Managers will work with individuals and families to develop and increase job seeking and employability skills to increase the potential for employment and self-sufficiency.
