Eligibility
Eligibility is determined by mulit-disciplinary evaluation team that includes the family, evaluator, service coordinator and others as determined necessary. The team reviews results of standardized testing, medical diagnosis and recommendations, other developmental assessments, childcare input and other resources that may contribute to a comprehensive assessment. The child is eligible if they have one of the following: a 25% delay in one area of development, a medical diagnosis that would likely cause developmental delay, or the informed clinical opinion of a qualified professional reviewing all of the above information.
Hours
Office hours: Monday through Friday, 8:00am to 4:30pm. Service Coordination visits and other IFSP services are provided at times and locations most convenient to families if at all possible.

Application process
Referrals can be made by the family or by doctors and others familiar with family concerns if they have consent from the family. To make a referral, call Bradford County Human Services and ask for Early Intervention. Upon receiving the referral, a Service Coordinator is assigned to the family. They meet with family/caregiver to gain a better understanding of the child and family's strengths, needs, supports and desires for their child's development. They work with family to plan the initial multi-disciplinary team evaluation to determine eligibility and to write an Individual Family Service Plan (IFSP) if the child is eligible for services.
Fees
There is no cost to families regardless of their income. All services are paid for using local, state, and federal funding as required by IDEA.

Families partner with their service coordinator and EI providers to build a plan based on their child and family's unique strengths and needs. Through coaching, families learn how to build opportunities for learning into their unique daily routines and activities, at home and in the community, The team providing services may include one or more of the following based on the family's individual needs: a teacher, speech, physical, and occupational therapists, vision and hearing specialists, behavior specialist and a list of other services.

Offers services and resources designed to support families of children birth to three years old, who have developmental delays or a medical diagnosis. Service coordinators and providers understand the importance of families leading their early intervention team. They guide their team in choosing what outcomes to work on. Families and caregivers work with teachers and therapists to embed learning opportunities into the child's everyday routines at home and in the community to promote their child's learning.
