Hours
Monday, Wednesday and Thursday, 8:00am to 6:30pm
Tuesday, 8:00am to 12:00pm
Friday, 8:00am to 4:30pm
Application process
Patients may be referred by a physician, insurance company, discharge planner/social worker, home health agency, family member, or self-referral.

Provides a variety of individualized outpatient programs, which may include physical therapy for patients with joint replacements, neck, back and shoulder pain, sports injuries, work-related injuries, orthopedic injuries, neurological disorders, pain management, hip and knee rehabilitation, arthritis rehabilitation, occupational therapy, hand rehabilitation, wound care, speech and language therapy, burn care, vestibular rehabilitation, dysphagia programs, lymphedema therapy and discharge planning.
