Description

Provides supportive services to older homeless adolescents. Services include: - Rent assistance - Basic apartment furnishings - Funds for food - Case management, life skills training and parenting - Clinical counseling - Transportation assistance - Utility assistance - Assistance with developing an Individual Service Plan for independence - Assistance with mainstream benefits qualification and application - Referral/linkage to other community resources/services
