Supports Coordination

Supports Coordination is the connecting bridge between the person with an intellectual disability/autism and the services and supports they need. Individuals with disabilities are assigned to a Supports Coordinator who locates, coordinates, and monitors services and supports for people with intellectual disabilities and autism.

Eligibility

Adults with an intellectual disability and/or autism diagnosis prior to the age of 22 or children under age 21 diagnosed with complex medical conditions
